Medicare pays for results

CMS pays Ciba based on health outcomes, not the number of visits. There is no separate program fee to you.

Is this program right for you?

The program is for people with Original Medicare (fee-for-service) who live with certain ongoing conditions. Taking part is voluntary, and it won't change your Medicare benefits. Medicare confirms whether you qualify when you sign up.

Clinicians - refer a patient

Do you care for patients with Original Medicare and ongoing chronic conditions? You can refer them to Ciba's ACCESS Model program. Your patient signs up and gives their own consent, you stay their doctor, and you receive updates on their care so you can keep coordinating it.

Is this Medicare Advantage?

No. This is a program under Original Medicare - the CMS ACCESS Model. It does not replace or change your Medicare.

What does it cost?

There is no separate fee to join, and Medicare pays Ciba based on your health outcomes. Your usual Medicare out-of-pocket costs may still apply to covered care, and we will always tell you any amount before your visit.

Can I keep my doctor?

Yes. You keep your own doctor and your Medicare benefits stay the same.

Is Ciba part of Medicare?

Ciba Health is a participant in the CMS ACCESS Model. The program is run by Ciba Health, not by Medicare.

How do I know if I qualify?

Medicare confirms your eligibility when you sign up. Taking part is voluntary and won't change your Medicare benefits.

What if my doctor told me about Ciba?

Your doctor can refer you, but you still sign up and give your own consent yourself. A referral never enrolls you automatically.
